I received a my heath pays rewards card in the mail about a year ago maybe longer.. recently I was on my insurance (buckeye health insurance) website and it says I have a little bit of money on the card for going to checkups and getting flu shots. On their website it says the card can be used at Walmart and Sam’s club for everyday items.
