Your midi-qol settings work in my environment. I'm using an adult red dragon attacking the Zanna PC from the compendium in the 5e system and it works. You should double check and update DAE, socketlib, and libWrapper. If that doesn't work, I suggest maybe backup and deleting settings.db in your world folder.
Take a look on the Midi-QoL settings, there's a configuration to automatically transfer effects on a failed save, that may not be selected. Try using some Midi-QoL/DAE SRD sample Item, that apply an effect, like bless, and see if they work.
